Initial medical examination
Analysis of anamnesis, blood pressure measurement, physical examination by a physician with a diet regime, drinking cure and treatments recommendation, including recommendations for any other examinations which may require prescriptions for medication.

The initial medical examination is intended for persons over 18.
It would be helpful (but it is not essential) to have any existing documents relating to specialist examinations (descriptions, results, lab examinations, ECG and others) you have had.
